About Pet Passports

Standard Pet Passports are $10 and come with a blank photo and information area where you will add your pets photo, name, date of birth, and additional information.
Custom Pet Passports are $20 and come pre-printed with you pets photo and information. The only additional information you would need to add, which I suggest writing in pencil, is on the Emergency Info page. When purchasing a Custom Pet Passport, please include the following information when replying to our Payment Confirmation email, along with your pets photo (see FAQ for photo requirements):

A Pet Passport is a great place to store all of your pets vital information should the need arise. Store information such as: Name, Date of Birth, Breed, Height, Weight, Sex, Tatoo Number, City License Number, Chip ID Number, Chip Service Provider Info, Registered Number, Sire's Name, Sire's Breed, Sire's Reg #, Dam's Name, Dam's Breed, Dam's Reg #, Veterinarian Name, Address & Phone #, Identifying Marks, Shot Records/Vaccinations. It also has important travel information should you actually decide to take your 4 legged family member with you.
*Pet Passports are NOT government documents, and do not guarantee entry into any foreign territory. Pet Passports are a novelty item only!
